USEFUL INFORMATION TO REPORT

• The exact address or location of the drug activity
• The color and detailed description of the residence where the drug activity is taking place
• The license plate number, color, make, model, year (as much information as you can safely gather) of any vehicles involved in the drug activity
• The names of any people who are known to be involved in the trafficking of illegal drugs.
• Time of day when the drug activity is most apparent.
• Are there any children who live at the residence or who are present while the drug activity is taking place?
• Any information about security at the residence (surveillance cameras, barricades, booby traps, etc.)
• Are there dogs, or other dangerous animals, kept at the residence?
• Are any of the subjects involved in the drug activity known to carry weapons?
• If known: What types of drugs are being sold?
• You may remain anonymous. However, if you are willing to assist our investigators or answer questions they may have, please provide a day time phone number where you can be reached.
